    Someone in management bought shares short, and now
    needs to drive stock down to make a profit. As only
    management cares about profit, whereas all unions only
    care about the comfort and safety of the passengers, it
    had to be management.

    Hmm. Tough. Can we blame the management of the company
    that runs the goods train?

  2. Management may be responsible in some of those examples. In a number of
    others I would suggest that it would be unreasonable for anyone to spend
    the money / employ the resources necessary to ensure 24-7 perfection and
    "zero risk" (whatever that is). All you would do is make railways far
    too expensive to build, maintain and operate thus resulting in their
    closure and a transfer of travellers onto less safe modes. Still we can
    wait for the hysterical reaction when we know what did happen.
