expose [the company] to penalties over $1M. The Root Cause Analysis
indicates both inadequate design for failure scenarios and lack of
production-like testing in any test phase." oops!

If they only had one guy to do design, coding and QA they've only got
themselves to blame. Mind you, that's what I do day to day, so I want to be
able to blame someone else when my time comes
