"In memoriam" donations In memoriam donations are commonly given direct by cheque or cash to the Charity Office, through the staff on the ward or via funeral directors. In all cases please let us know (preferably in writing) in whose memory the donation is being given. Please also provide your name and address so that we send you a receipt and acknowledgement. Please use the individual donation form attached to this email (PDF format) Completing a simple Gift Aid Declaration form enables the charity to reclaim 25p in the pound back from Inland Revenue. Donations to be eligible must relate to a donation given by an individual donor who has paid sufficient tax in the current tax year to equal or exceed the tax rebate that would thus become reclaimable to the charity.
