  2. The cables from the both engines fire detectors, vibration detectors,
    engine temperature etc, route together and have plugs that connect them
    to the main cable form. The plugs from the two engines were at that time
    not marked and were interchangeable.

    The plugs in the Kegworth crash were badly damaged so nothing could be
    proved. But in Boeing's subsequent investigation (a sort of maybe we
    should have a quiet look at our 737s) found aircraft with 94 with these
    cables cross connected.

  3. All personnel who worked on Ariane 4 at the pad ELA2 had to do a safety
    course and observing certain rules:

    During the last stages before the maintenance tower was rolled back:
    Always parking your car towards the exit with the keys in (and obviously
    unlocked. Nobody did.

    The other thing was the practice evacuation of the maintenance tower.
    The top deck is over 50 meters up. To get out in a hurry you jump in to
    the top of a vertical cotton sleeve. You have to trust that the cotton
    will pull tighter around you the faster you drop.

    First time is quite an experience.

  5. Imo, it's a contrast of carrying out his job/duty with professionalism
    in comparison to say, a crew of Egyptians on a ferryboat who will, as
    has been attested to several times, throw passengers to the deck and
    grab the lifejackets for themselves before fucking off over the side.

    I half remembered "something" about the fire control system being a bit
    dodgy, but it's a lot clearer now.
