There's another *slight* possibility (though the symptoms aren't quite the same): do you have mixer taps? My old house had a problem where the cold mains pressure was so much higher than the hot pressure (actually in the tap mixer) that if the cold tap was turned on too high, cold water would get pushed back up the hot pipe into the hot water tank.
That happens when all the imported made-in-France /Italy mixers get fitted to UK homes with tank systems. Those mixers are designed for equal pressure feeds, usually mains pressure. The traditional British mixer didn't mix at all until the water was actually exiting the spout, so could cope with widely differing pressures.
I've had them (BG) here for some years. They upgraded the system here with a condensing boiler. They've maintained it without complaint for 5+ years now despite having to replace the main hot water tank with a leak and it taking them a day to replace. They come when I ask and do the work they agree to. I can't fault them... yet!
